Cellular/Data Service
You have indicated that you are interested in obtaining a cellular phone and/or data service. The following forms will need to be filled out and approved by your Supervisors and Division Managers.
For New Requests for cellular phone/data, the New Service Form (see provider links below) must be filled out in addition to the following attached documents:
- Cellular Phone Approval
- DOA Acknowledgement Policy
- DOA Policy
- The Supplemental Form for Wireless Service (data phone only)
New Service, Upgrades, Changes, Cancellation Form Links

Complete the New Service form: Choose your plan, device and accessories. If you have questions contact your provider account representative. This form should be emailed to rhonda.loger@uwex.edu. DO NOT EMAIL TO YOUR PROVIDER. You also need to print the US Cellular/Cellular One forms and attach a copy to the supplemental form, etc. and obtain approvals. All forms then should be sent to the Purchasing Office. Once your approved forms have been received they will be matched with your email and your request will be processed. Complete the change, upgrade, or cancellation form: Fill out all information and follow process for New Service form above.
- All phones and voice/data plans must be purchased through Rhonda Loger, our Purchasing Director.
- UW-Extension Information Technology Department recommends a National Plan for persons that travel statewide.
- When your phone/data phone is received, contact UW-Extension Purchasing by email and provide the cellphone number assigned and account number.
- Also, when your data phone is received, contact the UW-Extension Information Technology Service Center by email at servicecenter@uwex.uwc.edu or by phone at 608-262-5034, as they will configure it to work with our Exchange email and calendaring system.
- Solutions outside of Windows Mobile O/S are not recommended.
