ADA Accommodations

Requests for reasonable accommodations for disabilities or limitations should be made prior to the date of the program or activity for which it is needed. Please do so as early as possible prior to the program or activity so that proper arrangements can be made. Requests are kept confidential.

Cancellation Policy

A 50% refund will be given for cancellations made by Sept. 1. No refunds will be issued for cancellations after Sept. 1 unless there is an emergency. Refund requests must be in writing and submitted to your State Delegation Coordinator.

Youth Health and Expectation Forms

All youth must complete a Medical Consent/Health Form and an Expectation Form. Please print the forms, fill them out, and send completed forms to your State Delegation Coordinator prior to the Forum. Following is more specific information.

Youth Participation: Information for Youth, Their Parents, and Adults Serving as Chaperones

A primary goal of this Forum is fostering youth adult partnerships.  The program has been designed for all youth and adult participants to be actively engaged together in all Forum events.

All youth participants must identify their adult chaperone when they register.  Chaperones must be at least 21 years of age. The Forum requires a minimum of 1 chaperone per 10 youth participants, and encourages an even lower youth-adult chaperone ratio, if possible. Youth participants and chaperones need to be in contact throughout the Forum.

Required for Youth Participation: Information for Youth and State Delegation Coordinators

If you are a youth, you must complete a Behavior Expectation Form and a Medical Consent/Health Form and return them to your State Delegation Coordinator before the Forum or you won't be allowed to participate. Most states and provinces, including Wisconsin, will use the two forms provided below. If your state or province chooses to use different forms, you will be notified by your State Delegation Coordinator.  Adults are not required to complete these forms. If you are not sure who to contact in your state, here is a list of Delegation Coordinators by State.

If you are a State Delegation Coordinator, please deliver the original of each youth's health form to Conference Headquarters when you arrive. Keep a copy of each health form and each original behavior form with you during Conference. You'll also be asked to provide a cell phone number or other contact information to Headquarters in case you need to be reached during Conference. When the Forum is over, you'll be asked to pick up the original health forms from Headquarters.
