Wisconsin 4-H Annual Charter Application: New 4-H Groups & Committees

All 4-H Groups and Committees that handle finances and have a checkbook or financial account must be chartered. 4 H Club Charters are issued when a new 4 H Group or Committee is formed. Charters are based on the completion of this “Wisconsin 4 H Annual Charter Application” packet. All sections in this eight-page packet must be completed by the group or committee leadership team and returned to the County UW-Extension Office or appropriate advisor.
