Is 4-H In The Title Of A Group In Your County That Is Not A Chartered Group?  Examples would be a 4-H Foundation, Endowment, Trust or Association, Camp or Fair.  These groups would be reporting to the IRS through a separate 501 c 3 or under another organization. These groups fall in the category of “affiliate” and are not a chartered group.  If you have such a group in your county or work with at the state level, I need to have their information at this time. 

The information needed is the same information that is requested in the Wisconsin 4-H Groups List.  They will be added into the chart that is submitted to the IRS.  The template for this information is attached.
