Self Initiated Performance Review (SIPR) Review

Creating Questions

Here are three examples demonstrating how you could create feedback questions from a content idea. There is no right or wrong way to do this. Any of these approaches can work successfully. Or you may choose another approach that you prefer.

Option 1: Scaled response Use the following rating scale for each questions;

NA-unable to judge
1-unacceptable
2-below expectations
3-satisfactory
4-very good

Mary Smith is knowledgeable about the subject matter. NA - 1 - 2 - 3 - 4
