It is time for 2010 Re-Enrollment for the Iowa County 4-H Program. You will see a new look to the Member Re-enrollment forms this year. All information we have is printed on your enrollment form. Please check the form over completely to be sure all the information is correct and we ask that you just check the projects you plan to keep and cross off the others.
Remember that youth in 3rd grade this year and were Cloverbud 3 last year have become full 4-H members and are able to choose from any of the projects listed on the attached sheet. Youth in K-2nd grade can choose from Cloverbud 1 – Cloverbud 3.
All re-enrollment forms are due to the UW-Extension Office by December 1, 2009. NO forms will be accepted without a leader signature. Check with your leader to see if they want the forms returned to them or if you can drop then off at the Extension Office. The enrollment fee must be submitted with enrollment form. The fee is $5 per youth member, or $15 for a family of three or more. Please make checks payable to: Iowa County 4-H.
Leader’s re-enrollment forms have the Behavior Form on the bottom so you only need to sign one sheet. You will need to have the club leader sign your form. Check with your club leader to see if you should return your form(s) to them or if you can drop them off at the Extension Office
If there are youth that have graduated from 4-H, parents or other adults interested in becoming a 4-H project leader, please complete a leader enrollment form and plan to attend one of the these volunteer orientation programs:
Thursday, December 10th 1-3 p.m. or 7-9 p.m. – County Board Room
Monday, January 11th 1-3 p.m. or 7-9 p.m. – Extension Conf. Rm
Wednesday, February 10th 1-3 p.m. or 7-9 p.m. – Extension Conf. Rm
March Project Learning Day TBA
All of these trainings will be held in the UW-Extension Office Conference Room in the Lower Level of the Iowa County Courthouse. If you plan to attend, please contact the UW-Extension Office to register at 608-935-0391.
