4-H RECORD BOOKS

Junior books are those submitted by members who were in grades 3-7.

Senior books are those submitted by members who were in grades 8-13 (one year past high school).

• Record Book Cover
• Record Book Evaluation Form (1 page, 22 KB) - This form is used by everyone. Any record book that is turned in must have one of these completed and placed on the front of the book.
• Youth Leadership Plan (2 pages, 110 KB)
• MPE-A Form - This form is used by everyone. Any record book that is turned in must have one of these completed and placed on the front of the book.
• MPE Form - This form is used by everyone. Any record book that is turned in must have one of these completed and placed on the front of the book.
• Dog Project Record (6 pages, 41 KB) - This form is used by everyone in the dog project. Each project needs to have one of these forms completed. The form is placed in the record book by the other project information.

• Horse Project Record (2 pages, 24 KB) - This form is used by everyone in the horse project. The Horse & Horseless Horse projects need to have one of these forms completed. The form is placed in the record book by the project information.

• Clothing Project Financial Record (4 pages, 50KB) - Required in all senior books in each of these projects:
  • 421-423 Clothing 1, 2 & 3
  • 486-487 Clothing Decisions 1 & 2
• Lifetime Dairy Record - Required to be completed in all senior dairy record books

  • 221-223 Dairy 1, 2 & 3

• Dairy Project Financial Record - Required to be completed in all senior dairy record books

  • 221-223 Dairy 1, 2 & 3

• Gardening-Photography-Woodworking Form - Required in all senior books in each of these projects:
  • 301-303 Photography 1, 2 & 3
  • 304 Videography
  • 581-583 Woodworking 1, 2 & 3
  • 721-723 Vegetables 1, 2 & 3
  • 731-733 House Plants 1, 2 & 3
  • 741-743 Flowers 1, 2 & 3
  • 751-753 Home Grounds 1, 2 & 3
• Meat Animal (Livestock and Poultry) Form - Required in all senior books in each of these projects:
  • 211-213 Beef 1, 2 & 3
  • 241-243 Swine 1, 2 & 3
  • 251-253 Poultry 1, 2 & 3
  • 261-263 Sheep 1, 2 & 3
  • 255 Turkeys
  • 256 Bantams
• Crops Form - Required in all senior books in each of these projects:
  • 701 Corn
  • 702 Forage
  • 703 Small Grains
