2008 PRIVATE PESTICIDE APPLICATOR (P.A.T.) TRAINING - Completed

YOU CAN STILL TAKE THE TEST!!!

Please contact the UW-Extension to schedule a time to take your test. The fee is $30. Please make your check payable to UW-Extension.


Please bring ther following items with you when you come to take test. 
(1) Photo Identification, (drivers license for example)
(2) manual
(3) your social security number
(4) a pencil
(5) a hand calculator is helpful and 
(6) you may eat out or bring a lunch for day-long sessions.  If you have any questions, please call me at 723-2125.
