Leader Enrollment

Filling out enrollment forms: It is very important that ll enrollment forms be filled out COMPLETELY and CORRECTLY to avoid problems with newsletter mailing and fair entries. If you have any questions on filling out the forms, contact the Extension Office.

All general, project, activity, and resource leaders need to complete an enrollment form.  All new leaders will need to complete a 2-hour YPP (Youth Protection Program) training as required by Wisconsin 4-H in order to be certified to chaperone 4-H events or to work one on one with youth.  There will be one/two YPP training sessions offered in the upcoming months.   Leaders who do not become YPP certified through the state of Wisconsin will not be allowed to be leaders in Monroe County 4-H.

Leader lists will be generated from the forms received.  Leaders names must be on file at the UW-Extension Office to be eligible for medical and liability insurance coverage. Leaders should list specific projects they will be leading.  You can list either a specific project or the project code for an area; i.e., Dairy Leader with project code 220 - OR - Computers with project code 373, for example.
