Member Enrollment

Filling out enrollment forms: It is very important that ll enrollment forms be filled out COMPLETELY and CORRECTLY to avoid problems with newsletter mailing and fair entries. If you have any questions on filling out the forms, contact the Extension Office.

Members should never use a project code ending in "0" unless they are signing up as a member with junior leader responsibilities.  These are reserved for leaders.  Because of the progressive nature of many projects, sign up for only one level of a project area per year (example: dogs, photography, woodworking, vet science, etc.) Members must be enrolled in a project in order to enter in that area at the county fair.

Help for New Enrollments

  • Each family should receive a Project Guide, one blank enrollment form (2 sided) per new member or leader enrolling, and Supplement Sheet (attached) from their club's enrollment chairman.
  • Write in your club name.
  • Do not complete county codes, club code or member code.
  • Circle "new" on enrollment form if enrolling for the first time.
  • Fill in name, address, phone number, birth date, social security number, etc.
  • Birth date for Cloverbuds, Explorers, and members is very important information needed for the Blue Ribbon Program (information is always kept confidential.)  For leaders, we need to have on file your birth date and social security number (optional), as this information is necessary in the certification of leaders through the Wis. Dept. of Justice.  All information such as this is kept confidential.
  • Accurately fill out project names and project codes.  Youth leader information for grades 7th–13th is optional.  Please circle yes/no on enrollment form after each project.  First year members are encouraged to take only one or two projects.  Record book forms should be completed for each project at the end of the year.
  • Members should proofread all information on enrollment form, change or correct wrong information.
  • Complete release of information section (photo release, disability accommodation, and public record) below the project name and project code box.
  • Enrollment Chairman must hand in enrollment forms corrected and complete (signed by member, parent/guardian, and leader), no later than November 15.  All changes/adds must be made by January 1.  New members will be entered in the computer by the Monroe County Fair entry deadline.  If a new member wants to exhibit in Junior Fair, they must enroll by April 1.  After this deadline, new members can only show in open class.  

Help for Re-enrollments

  • All cloverbuds, explorers, members, and leaders who were enrolled previously will receive an enrollment form with their name on it.  If you do not receive a personalized form, complete a blank enrollment form.
  • Check name and address and make corrections as needed.
  • Fill in any blanks.  SCHOOL and RESIDENCE should be completed.
  • Check CATEGORY.  It should read "member", "cloverbud" (includes explorers), "project leader", "general leader" etc.  If it is incorrect, write in correct category name.  Third graders becoming regular members should change "cloverbud" to "member".
  • IMPORTANT!  All members and leaders quitting 4-H because of graduation, lack of interest, moving away, etc. must return their form so the computer can be updated.  Enrollment Chairman should write "DROP" on these forms.
  • Fill in SOCIAL SECURITY NUMBER (optional).
  • GRADE & AGE should be as of January 1, 2007.  Check YEAR IN 4-H and GRADE to make sure they are correct.  Cloverbuds and Explorers YEAR IN 4-H is “0”.
  • Explorers becoming regular 4-H members will be first year members.  YEAR IN 4-H will be “1”.
  • Leaders should keep track of the number of years they have been a leader on the YEAR IN 4-H line.
  • If a member or leader is changing clubs, they should be given their personalized re-enrollment form.  Write in the name of the new club under CLUB MEMBERSHIPS.  The re-enrollment form should be submitted to the general leader of your new club for signature. 
  • Fill out project names, codes, and youth leaders information accurately on enrollment form.  PROJECT LIMIT IS 24.  Remember, for every project you take you will be expected to complete a record book form.
  • Members should proofread (check for accuracy) all information on enrollment form.  Correct wrong information.  Enrollment chairman should hand in enrollment forms for re-enrollments, corrected and complete (signed by member, parent/guardian, and leader), by November 15.  All changes (drop/adds) to projects should be made by January 1.
