ACCESS
serves to coordinate resources and programs
that assist persons in crisis. Crisis being an acute change in
the status of individual and family stability resulting in a request
for immediate intervention.
ACCESS
GOALS:
- streamline
the intake process on behalf of clients
- more effectively
cooperate to allocate resources and meet client needs
- supporting a circular continuum
of care
- more appropriately
make referrals and reduce paperwork
- increase ability
to collect local data on human resource needs and housing
- use data to
access outside funds and grants and increase program accountability
ACCESS PARTNER
PARTICIPATION: :
- attend training
workshop and six month trial of the ACCES S process
- identify clients
who have multiple needs/in crisis
- initiate contact
with other partner agencies that can assist with needs
- communicate
with other agencies to assist with meeting needs
- share client
documentation with other helping agencies
- provide client
data to data base
In the short-run ,
ACCESS collaboration be done through email, telephone
and face-to-face meetings until grants provide for a web-based
ACCESS system.
