Evaluation Forms and Instructions

Sharing Success Stories
Evaluation is vital to the continuing success of the Powerful Tools curriculum. By ensuring that class participants complete the following evaluations, you provide the data needed to show that Powerful Tools workshops change lives - for caregivers and care receivers. Thanks to you, we're able to share the benefits of Powerful Tools programs throughout Wisconsin.

Pre-evaluation and Post-evaluation
These evaluations tell whether participants:

  • Liked the class
  • Used the tools
  • Read the Caregiver Helpbook
  • Benefited from the class by feeling better about themselves, becoming more competant caregivers and using strategies to help them as caregivers.

6-month followup
This evaluation gives feedback on the sustained benefits of Powerful Tools. Using the same questions as in the pre-and post-survey, this survey allows us to compare data on behavior, emotion and preception at three points in time.

Collecting Data - Tips for Class Leaders
In conducting evaluations we recommend the following procedures:

  1. Mail pre-evaluations with confirmation letters and ask registrants to complete the forms and bring them to the first class.
  2. If participants fail to bring the forms, give them another copy to omplete before they leave.
  3. Give post-evaluations at the end of the fifth class and ask participants to bring completed forms to the final class or give evaluations at the last class with return envelopes. If you do the latter, track returns and call, send postcards or new evaluations to non-repondents. Another option is to take some time during the last class to have participants complete evaluations.
  4. Code each individual's pre-evaluation with an identifying mark as soon as you receive it. An identifying mark can be as simple as a number, letter, combination of numbers and letters, or any code you choose. List identifying marks on a "code sheet" (with particpants' names and codes) so you can use the same codes on participants' post- and six-month evaluations with the same identifying marks as soon as they are turned in. Once all are coded, destroy the code sheet.
  5. To ensure a good return rate, keep track of returned evaluations and send a postcard or another evaluation to individuals who fail to retrun evaluations within three weeks.

How to Enter Class Data
You can input pre- and post-evaluation data in one of two ways:

  1. Enter class data yourself and follow data input instructions.
  2. Send data to UW-Extension (Mary Brintnall-Peterson, 432 North Lake Street, Room 301, Madison, WI, 53706) and we will enter it for you. If you choose this option, you need to send coded pre- and post-evaluations for every participant who completes an evaluation. Complete the Enter class data form and include it when mailing your evaluations.
