EVALUATION NOTE

  • Each facilitator must acquire his/her own human subject’s protection approval. Localize the human subject’s protection statement including your name and contact information

ADULT SESSION EVALUATION

  • Print copies of the evaluation (by clicking on the link below) and provide time for participants to complete the evaluation at the end of the program.
  • Compile and summarize the evaluation using the Microsoft Excel template.
  • Send an electronic copy of the Excel file to Donna Doll-Yogerst via an e-mail attachment.
  • Look at the evaluations, both individually and the summary, to understand successes of the program and where it may need to be improved.
  • 3, 6, 9, or 12 month follow-up/post evaluation same as above and also send the Excel file to Donna Doll-Yogerst via an email attachment.
  • Use the Success/Impact story template (link below) to share your program results.
  • Please join the State and National impact report!

Follow-up/Post Evaluation (2 versions available)

If using the mail follow-up/post evaluation indicate they will receive a survey in the mail with a postage paid return envelope in 3, 6, 9, or 12 months (whatever your plan is) and would appreciate their comments at that time.

Telephone survey:

Have a stack of postcards ready at the last session. Talk about doing a follow-up survey and ask people to write their name and address on a postcard if they are interested in participating in the follow-up survey. A week before you plan to do the survey, send out the postcard. Here is an example of the text you could use on the postcard "We are interested in following up with you regarding the strategies used for parenting your spirited child. We will be calling you to talk more about your spirited child." Make sure you provide your county identifying information. You may also want to let participants know the date and time you will try to call.

CHILD SESSIONS EVALUATION

The Child Sessions offer two evaluation tools:

Teacher Evaluation (MS Word document)
This is a summary of each children’s session. The teacher is to fill in the children’s response to their round robin question, “What tip or tips that we talked about tonight are you going to try to use tomorrow or sometime this week?” Return the tally to the facilitator of the adult session.

Parent Postcard Evaluation (MS Word document)
This postcard is distributed at the parent session. Coordination of this evaluation between the adult and children session facilitators is important. Distribute the parent’s response (prepaid will generate more returns) postcards in the adult session. Ask the parents to complete the postcard in a day or two and return them to (insert who you would like to receive the postcards).
