Home Visitation is a family support service delivery model designed to improve:
family functioning;
promote child health, safety, development and school readiness; and
prevent child abuse and neglect.
Key home visitation program elements include:
early identification of pregnant women and families of newborns through initial screeening and assessment (often by trained paraprofessionals),
psychosocial support,
parenting and health education,
nutritional counseling,
individual planning with the family to build on their strengths and work on family-identified goals, and
assistance for access to needed community resources.
