then mailed. Please print a copy of the
completed form for your own records.
Payment for entering these trials cannot be made on-line as of yet. Payment MUST still be mailed the
old-fashioned way.

Update for Wisconsin” booklet,
published by the University
of Wisconsin-Extension, once the initial trials have ended, they must
be re-entered in at least two
