University of Wisconsin Variety Trial Forms

All entry forms are in Adobe Acrobat (PDF) format.  If you do not have it already installed, you will need to download and install the free Adobe Acrobat Reader software in order to view the Adobe Acrobat forms.  These forms are to be printed, completed, and then mailed.  Please print a copy of the completed form for your own records.  Payment for entering these trials cannot be made on-line as of yet.  Payment MUST still be mailed the old-fashioned way.

For commercial alfalfa varieties to remain in the “Forage Variety Update for Wisconsin” booklet,

published by the University of Wisconsin-Extension, once the initial trials have ended, they must

be re-entered in at least two sites.  For a listing of those commercial alfalfa varieties that will be

dropped from the Forage Variety Update for Wisconsin booklet unless re-entered,
