Instructions for submitting
county-level data on worksite or restaurant smoking policies
PLEASE NOTE: CHPPE will not provide
data analysis to local coalitions. The data will become part of
a statewide dataset, but coalitions will not receive individually
analyzed data.
As you may know, the WTCB Monitoring and Evaluation Program has
contracted with UW-- Madison's Center for Health Policy and Program
Evaluation (CHPPE) to conduct a statewide evaluation of tobacco
control activities funded by the WTCB. As part of this statewide
evaluation, CHPPE is requesting that coalitions who used the MEP
worksite and restaurant questionnaires submit their data to a
central data file. CHPPE will analyze the local data for comparison
to data collected through statewide, random sample surveys. Your
data will also add a dimension of richness to the statewide evaluation
by providing a much more detailed look at policies at the local
level.CHPPE will combine the data into one database. To facilitate
confidentiality of respondents, please strip your dataset of identifiable
information, such as respondent names, names of worksites, addresses,
or telephone numbers. It is appropriate to include names of cities,
towns or villages, zip codes, and the name of the county.
To submit your data to the statewide evaluation dataset, please
complete the form below and submit it along with an Excel
or Access data files with this complete form to Amy Anderson at
aeanders@uwccc.wisc.edu.
Please complete a separate form for each data set you submit.
