Evaluation Questions & Forms for Use with Specific Lessons or Curriculum (FY12)

Situation

• Many of the teaching activities that take place as part of WNEP/SNAP-Ed are taught using teaching resources or curricula that have been designed specifically for food stamp-eligible audiences and include an appropriate evaluation design and tools.
• The evaluation tools that are designed and tested in association with the specific associated teaching resources should not be used with other lessons or teaching events.

Evaluation Tools

These are a variety of evaluation tools to be administered as instructed in the lesson or curriculum. Please be sure that all related guidelines and protocols are followed as written for the teaching resource.

Do not change the wording of the questions on the tools or surveys!

Human Subjects Protection—Informed Consent to Participate

The end of session evaluation questions that are designed for this evaluation project are intended to be asked of all willing adults who participated in the lesson being evaluated.
Before asking the end of session evaluation questions, say the following to the learners:

"I am going to ask you a question that will help us see what you have learned today. You do not have to answer the question if you don’t want to. All of your answers will be private."

Do not record individual learner’s names on evaluation forms or questions. Do not share individual answers or comments made by the learners with other teachers or staff.
