Driver Authorization Procedures

In order to be covered by the State's Liability Protection Program while driving any state vehicle on university business (whether controlled by the university or by another state agency), all prospective drivers must have driver authorization. UW-Madison Risk Management has been reviewing driver records and processing vehicle authorizations requests for UWEX employees, students, LTE's and volunteers for many years. To get driver authorization, please complete the appropriate driver authorization request form (see below) and submit for processing (follow instructions in the form).

  • Faculty and staff must complete a Vehicle Use Agreement. This form indicates that the driver has received and understands the Fleet Driver Policies and Procedures and Appendix, and UW-Madison Fleet Rules and Regulations. A check of your motor vehicle record will be made. Once approved to drive, employees will be listed on the Approved Driver Database. If not approved, the requesting department will receive an e-mail rejection from the UW-Madison Risk Management office. Specific information regarding the process for authorizing faculty and staff is available in the Faculty/Staff guidelines.
  • Students, LTE's and volunteers must complete the Student, LTE, & Volunteer Driver Authorization form. Again, a check of your motor vehicle record will be made and, if approved, the drivers name will be listed on the Approved Driver Database. If not approved, the requesting department will receive an e-mail rejection from the UW-Madison Risk Management office. Specific information regarding the process for authorizing students is available in the Student/Volunteer Guidelines.
  • In both cases, if the driver has an out-of-state license, or a Wisconsin license for less than 3 years, the driver must complete a Notarized Statement of their driving record and have the form notarized, before submitting it along with the appropriate application.
  • Due to the State's increasing scrutiny of driver qualifications, UW System will no longer accept driver licenses from other countries, Canada excepted. Individuals will need to contact the Wisconsin Department of Motor Vehicles to obtain a Wisconsin driver's license before being authorized to drive. From this point forward, UW-Madison can only accept licenses from the US, it's territories, and Canada.
  • When hiring for a driving position, please fill out a Pre-Hire Driver's License Check form. Risk Management will do a license check and respond before the employee is hired. This process applies to Wisconsin licencees only.
  • 12/15 Passenger Vans, the University does not recommend the use of 12/15 passenger vans if other methods of transportation can be used. Rules now stipulate that all drivers must be at least 25 years of age, have completed the appropriate van-driver training course, and meet all other driver requirements. Van training is offered through the State Van Pool office at 266-5612 or online at www.doa.state.wi.us/training/training_reg.asp?classid=233. If you take the class, please inform the UW-Madison Risk Management office of satisfactory completion by mailing or faxing a copy of your van driver training card (fax# 608-262-9082). PLEASE, NOTE: the van driver training expires after three years from date of completion.
