Fulfilling the Promise 2012

Home Visitation is a family support service delivery model designed to improve:

  • family functioning;
  • promote child health, safety, development and school readiness; and
  • prevent child abuse and neglect.
Key home visitation program elements include:
  • early identification of pregnant women and families of newborns through initial screeening and assessment (often by trained paraprofessionals),
  • psychosocial support,
  • parenting and health education,
  • nutritional counseling,
  • individual planning with the family to build on their strengths and work on family-identified goals, and
  • assistance for access to needed community resources.
